Qualtrics You Online survey tool that allows one to build surveys, distribute them, and analyze responses. Log into Qualtrics using your WSSU login creditials.  Survey results can be set to be emailed to various different parties. Data can also be reviewed in both the Data & Analysis tab and the Results tab within a survey project's menu.
Microsoft Forms You Easy to learn, do-it-yourself form creator. You oversee creating this form and can interact with the results without needing to contact a 3rd party. Login to Microsoft Forms using your WSSU credentials. Training for Microsoft Forms is available on the IT Training Calendar. The form submissions are emailed to the form creator (or anyone else you choose). Form data is also stored on an Excel sheet that you can access, edit, and share.
Wufoo Application Administration Services Wufoo forms integrate well with the WSSU website and can be branded to maintain an official WSSU look. A Wufoo form must be requested by submitting a RAMTech ticket to Web Support Services. Complete the "Create a Web Form" Request in the RAMSupport - RAMTech system 
  • Individual form submissions are emailed to one or more email recipients of your choosing.
  • An excel spreadsheet that contains all available form data can be requested via RAMTech ticket.
  • A dashbaord can be developed of the resulting data.

Adobe Sign Application Administration Services Adobe Sign allows you to securely make agreements and approvals using a digital medium. An Adobe Sign sending account can be requested via RAMTech. In Adobe Sign, all parties are emailed a copy of a form or document once it has been signed. For web forms, in addition to the data being emailed, a spreadsheet of all submitted data can be found by going to the Manage tab, selecting Web Forms, opening the specific form, then choosing Download Form Field Data from the Action menu.
Fillable PDF You do! Fillable PDF forms allow for a mixture of electronic or physical responses. PDF forms must be ADA compliant to be published online. You will need Adobe Acrobat Pro software. Request it via RAMTech Support. For training on making your PDF files ADA Compliant, please see the IT Training Calendar. Completed PDF forms can be emailed to you by the user or printed out and turned in in-person.

To provide the user with basic information about the applications used to create forms so that they can make an informed decision.
