Standard Withdrawal Request

This form is used to request a standard withdrawal from one or more courses or from the University during the regular withdrawal period published in the Academic Calendar.

You should submit this form when the withdrawal request is not related to an extenuating circumstance and is being completed within the approved withdrawal deadline for the term.

Undergraduate students should be aware that course withdrawals completed after the census date count toward the University’s maximum of 16 credit hours of permitted withdrawals without extenuating circumstances. Students who exceed this limit may be required to submit a Insert Link to Withdrawal Due to Extenuating Circumstances Form request instead.

To complete the form, you must provide the term of withdrawal and course information (if applicable). Once submitted, the request will be routed to your academic advisor and/or department chair for review and processing.

If approved, a “W” grade will be recorded on your transcript. Withdrawal may impact financial aid, academic progress, housing, and financial obligations.
