Supervisor Change Request

Service Overview:

The Supervisor Change ticket is used to update the assigned supervisor for a specific employee or team. This update supports effective management, oversight, and team continuity, while ensuring HR and payroll systems reflect the current reporting structure.

This request is initiated due to [reason for change, e.g., promotion, department transfer, resignation, or organizational restructuring]. It must include the new supervisor’s information, start date (if a new hire), position number, names and Banner IDs of all affected employees, and the effective date of the change.

Please note: A separate Supervisor Change request is required for each unique supervisor and employee group combination.
