Service Overview:
In order to determine the eligibility of a student pursuing a second Bachelors's degree for financial aid (Title IV, institutional & state) the Office of Scholarships and Financial Aid needs to be able to verify applicable courses. Please ensure that the information is accurate and contains proper signature(s). Incomplete forms will not be reviewed and will result in a delay of financial aid. For auditing purposes, course listings cannot be attached, they must be recorded on this form.
